feat(chain): add txout methods to KeychainTxOutIndex ...

`txouts` and `txouts_in_tx` are exposed from `SpkTxOutIndex`, but
modified to remove nested unions.

Add `keychain_outpoints_in_range` that iterates over outpoints of a
given keychain derivation range.
